I just wanted to let the USA record applicants know that the most
current version of the AMA Indoor record application form is located here:

If possible I would like to have the current forms used when you set a
new record. I have been getting some old versions of the forms and those
old forms have some errors on them so if everyone can pitch out the old
ones they have and use the current version that would be appreciated!
