Construction notes

In some older issues of INAV I see plans accompanied by 2 or 3 pages of
construction notes. These notes are on a standardized sheet with
information like wood grain, density and weights all organized by
component. Does someone have a blank copy of these construction notes that
they could scan and post? It's much nicer than the mess of scribbles I
have in my various notebooks.
