The bad news is that the welcome center folk are notorious for losing previously delivered background check forms.

 The good news is that they can now go through the whole process with a new form in about 10 minutes.
 

 As long as you have a completed form (just in case they don't have your old one), and get there with plenty of time before closing, you should be fine.
