I would write in the event on the correct form, create a box, and check it.
There is not a second, corrected mailing, but the corrected form should appear
on the AMA events website for use between now and then.
The AMA publicist who created it is Yolanda Jones. Her e-mail has changed since
last year. Often she copies most of the form from the previous year, which may
be what has happened.
