USIC entry fee

Hi Folks,

When I updated and distributed the initial version of the 2006 AMA Nats
entry form, I didn't know that the Nats Planning committee voted to
increase the entry fee from $30 to $40 (This error also slipped by AMA
at first, too). The entry was corrected on later versions.

If you were conscientious and registered early with my erroneous form,
please excuse my mistake. If you can bring an extra $10 to help us
balance the books, it would be greatly appreciated.
